Negotiations over hospital staffing

Portuguese Health Minister Ana Paula Martins met with representatives from the Association of Service-Providing Doctors (AMPS) on Friday morning, 2 October 2026, for talks that lasted over three hours. The session began at 10:00 without a detailed advance agenda from the ministry, taking place nearly one year after the previous formal discussion on 27 November 2025. AMPS representatives brought several operational proposals aimed at improving resource management across the National Health Service (SNS), particularly in hospital emergency departments. While both parties narrowed their differences regarding career integration, the meeting ended without a comprehensive agreement due to outstanding disagreements over professional pay rates.

Career paths for non-specialist doctors

Discussions focused primarily on creating a legal framework to transition non-specialist contract physicians into permanent SNS staff positions. Under existing Portuguese legislation, public health institutions cannot grant indefinite contracts to doctors who do not hold a medical specialty, a restriction that excluded non-specialists from the three-month transitional hiring window that expired on 30 September 2026. Martins indicated willingness to establish dedicated access routes to specialties, allowing physicians to qualify through standard residency training or through formal certification consensus with the Portuguese Medical Association (Ordem dos Médicos). AMPS leadership confirmed plans to request a direct meeting with the Ordem dos Médicos to coordinate the regulatory requirements for these credentials.

Impasse over hourly pay rates

Although the morning discussions avoided the topic of hourly compensation, fee scales remain the central point of contention between the government and independent practitioners. A joint ministerial decree from the Ministries of Finance, Public Administration, and Health is due for publication during October 2026 to define official remuneration per hour, per medical task, and per emergency shift schedule. AMPS demands a baseline rate of at least 43 euros per hour, and association leadership warned that any government attempt to lower current compensation levels could lead to mass departures and service stoppages among contract doctors.

Reliance on outsourced emergency staff

The negotiations follow a decree-law published in June 2026 to regulate medical service procurement across public healthcare units. Local Health Units (ULS) rely on outsourced physicians, commonly known as tarefeiros, to cover emergency shift rotas and address specialist deficits in their permanent staffing rosters. AMPS reported that initial contract proposals presented by local health boards were turned down because they offered pay equivalent to first-year interns with no career progression, despite the demanding nature of emergency work. A June report from the Public Finance Council noted that outsourced medical procurement retained a substantial financial share in 2025, reflecting persistent limitations in the direct staffing capacity of the public health system.
